Question:

I have a small spot on my living room ceiling approx. 4 in. in diameter, & the paint is coming off. Would it be ok to remove the loose stuff & patch it with drywall filler or do I use tape & then the filler

Answer:

If the gypsum board has not mushed away, you can prime and paint but first you have to find out why the spot got wet in the first place. If it needs repair, you'll have to use tape and filler.
