The painted cement floor is cracked and uneven from settlement. Can you recommend a brand of leveling material and a modern finish material for installation over the concrete?

Most industrial chemical companies put out a good leveling compound but I don't know that you'll need it. Basement floors are poured off level so flood water drains to the lowest spot and I don't think we should monkey with that.
